To best assist you, please have the following information nearby when submiting your information:

  1. Names, addresses, phone numbers, and date of birth of all children;
  2. Name and address of sucessor trustee and alternate successor trustee;
  3. Names and address/phone numbers of any assets you desire to place in the Trust (e.g., bank accounts, investment accounts, etc.)(NOTE: Insurance benefits, 401K, and pension funds are best not put in Trust)(ALSO: We will assume all these assets are marital community property if you are married. Please tell us if they are owned by one partner in the marriage);
